COVID-19 Pop-Up Testing in York Centre: Tuesday, January 26

Dear Community Member(s),
SickKids and Women’s College Hospital are pleased to offer COVID-19 testing in your community. We have highlighted information along with detailed instructions to improve the speed and flow of the process on the day of testing. We have included a couple of required forms with this email to fill out in advance of your test.
Testing Date: Tuesday, January 26th
Location: 475 Patricia Ave

Pick up and Drop off times for Saliva (spit) and/or NP (nose) testing as follows:

• 9:30 AM – 12 PM
• 1PM – 4:30 PM
• 5 PM – 7 PM

IMPORTANT INFORMATION

• Please fill out all information on the “COVID19 Testing Information Form” for EACH family member being tested
• Please email this form to mobile.outreach@sickkids.ca by Tuesday, Jan. 26 at 7:00 AM (Note: if you miss the deadline, please print out the forms and bring them along).
• Please see attachment “how to access test results” for result information
• Please remember, you MUST wear a mask inside AT ALL TIMES and respect physical distancing rule

INFORMATION FOR PICK UP (~5-10 minutes)

• ONE person per family will check in with an admin person in the PICKUP room. If you emailed your form(s) in advance, they will have you on the list. If you did not email your form in advance, bring in a printed copy of the completed form.
  • Step 1: Our Admins enter all of the data before you move on to the next stage.
  • Step 2: We have SALIVA testing for those not present on site and NOSE swabs for those onsite
    • SALIVA testing is available for those 4 years old and up who can spit into a cup.
    • If you have child(s) under 4 years, please bring them with you for a nose swab.
    • We recommend the person who picks up all the Saliva kits does a nose swab
    • Please bring the “REQUISITION FORM” filled out at Pick Up when doing a nose swab
• We are unable to accept large families into the testing space due to capacity restrictions
• Important reminder for saliva tests: You CANNOT eat, drink, chew gum, smoke or vape 30 minutes before collecting the sample. PLEASE do collect saliva sample inside the bathroom or inside the popup space. We recommend you go in the car or back home to collect the sample.

DROP OFF (~5 minutes)

1. Please walk to the room labeled DROP OFF.
2. Check in with an admin person – let them know the names of whom you are dropping off spit samples
   • It is helpful if you write down all the names of your family members in advance to help us locate the names on our list
   • Include the “REQUISITION FORM” filled out for EACH saliva sample (in the outside pocket of the bag)
3. You are then free to leave – THANK YOU for participating in our community testing!
